If you wish to pursue your own behavior modification project, correctly termed self-modification, please use the following worksheets. Next to each is an indication of which module you can review to better understand how to complete them. If you were developing a project for someone else, you can use the same sheets. Instead of referring to yourself, refer to the client.

The proposal brings together all the previous sheets which build out your plan. This is one way to do the proposal but you need a proposal to formalize everything.
During the proposal planning you would conduct the baseline phase to be able to complete the functional assessment.
Once the proposal is complete, you can begin treatment phase. You will need the documents below for it.
